Acceptable Advertising Material: Film negatives; finished artwork or mechanicals are acceptable. (Conversion of these materials will be rebiled at cost.) All materials furnished will be destroyed by Quest Systems, Inc. if not requested within 13 months after last used.

Issuance and Closing Terms: ("SMALL PRINT" !!! READ IT, PLEASE!)

Advertising closes July 1, 1996 (for Regular Edition).

Neither advertiser nor its agency may cancel after closing. Cancellations (as well as changes) will not be accepted by Quest Systems, Inc. (herein called 'Quest') after closing date. Cancellations must be in writing, and none is considered accepted until confirmed in writing by Quest.

Quest reserves the right to reject advertising which it feels is not in keeping with the publication's standards.

Quest reserves the right to select or change ad business classification.

Quest shall not be responsible for loss of profit, loss of business or any other consequential damages as a result of any error or omission in or of an advertisement.

Quest is not liable for any failure to publish or circulate all or any part of any issue because of circumstances beyond its control.

The advertiser and/or the advertising agency for publication assumes sole responsibility for the protection of its copyright in any writing, pictorial illustration, design, map, photograph, or combination thereof included in its advertisement(s).

The applicant agrees to indemnify and hold harmless Quest from and against any and all liability, claims, demands, suits or causes of action whether or not attributable to the negligence of the Quest, and will pay all expenses including attorney's fees, incurred by the Quest in the defense thereof, arising out of the publication of any trademark, trademark name, or any name, portrait, picture or illustration of a person in accordance with this order or any copy submitted in connection with this order.

The applicant agrees that Quest's liability for damages arising from errors or omissions in the making up or printing of its directory listing or advertising shall be limited to the amount to be charged for such directory listings or advertising.
